Understanding Income withholding forms

I have just received my income withholding form for a new job. They state that they will be taking one amount for Past-due support. Then on the last part of the order information they are taking a higher amount for arrears and fees. I figured that they where the same.

Also I have two cases but one is want to take 50% of my income. Is that something that they can do even though that they know I have another case?
