Supervised Visitation

Under what circumstances will the courts consider supervised visitation of a child with the non custodial parent? We currently have no scheduled visitation arranged. My daughter is 3 years of age and her father has seen her a total of two times in her life with the last time being 2 years ago in May of 2010. I also wanted to know is it possible for guardianship to be granted to another individual other than the father if the mother were to pass away or something of that nature at the request of the custodial parent?
