NC Parent And Medicaid

In 2005, my son was notified that he had a child that was recently born and was very ill with birth defects. A short time there after, a paternity test was completed and proved he was the father. This paternity test was not an order of the court, but purchased and provided by other sources. At the time of birth, mother was previously employed but when the child was born and due to the medical issues of the child, the child was automatically placed on Michigan's Children Special Health Services and SSD (for disabled children)and she no longer worked. Mother also received child support from father since birth.
My questions are:
1-Can the state order father to pay back for all medical costs paid by the state where it was deemed the child was born with birth defects and is special needs? I recall somewhere that children born with severe birth defects are automatically placed on Children's Special Heath Care. In other words, the State automatically steps in when parent (s) are low income (please correct me if im wrong). In other words.. severely disabled children born in Michigan automatically get state insurance?
2-If both parents are low-income, and have been since the birth of the child, will they still order him to repay what the state has paid?
3-It is obvious to us that the state has paid into the thousands, maybe even million for the necessary care for this child. What does the state do in extreme cases like this?
Aside from coming into funds by way of lotto or some venue of the sort..I dont see how can he ever pay it back.
